Real, persistent pain of headaches - There are many different types of headache, each with its own triggers

Real, persistent pain of headaches
By WONG LI ZA

There are many different types of headache, each with its own triggers. DERRICK Chen thought nothing of having two roti canai for supper a few weeks ago. He went to bed not long after, but in the morning, what greeted him was a sharp pain on the right side of his head.

This was a pattern he noticed lately: a heavy supper preceding a pounding headache the next day.

Chen, 41, has been suffering from headaches for the past 10 years. He describes his pain as “pin prick” in nature that occurs on his left or right temple. Sometimes he experiences a dull, pulsating pain at the back of his neck.

“If I take two (paracetamols) at the start of a headache, I will be fine the entire day. However, the following morning, the headache returns and I will need to take two more painkillers,” said Chen, a businessman and father of three.

His longest spell lasts seven days; his shortest, one day. On average, Chen, a smoker, experiences two to three headaches a month, usually migraines.

Take the pain away: Preventive measures, from medication to lifestyle changes, can help most people who suffer from migraine headaches

So far, he has seen two neurologists and undergone a magnetic resonance imaging (MRI) test, but doctors found nothing.

“They told me it’s stress, to take it easy and continue taking paracetamol. One doctor said it could be my eyesight but I had that checked and it was fine.”

Chen has also consulted a traditional Chinese sinseh who prescribed him dried cockroaches boiled with herbs to drink.

“I’ve tried that three times but the headaches are still there,” he said.

Peanuts may trigger headaches in some people.

Sakeenah (not her real name), started getting migraines when she first returned to Malaysia from Britain 20 years ago.

“It was most probably due to the heat,” recalled Sakeenah, who was in her mid-30s then.

She suffered from migraines once a month, especially on the first day of her period. She would also get a migraine if she missed a meal or when travelling overseas.

“I would get this throbbing pain on the right side of my head, but the pain was not so bad then,” said Sakeenah, now in her mid-50s.

Her migraines worsened when she went into menopause in her early 50s.

“I would get it once every two weeks and the pain was much more intense. Work stress added to the intensity and late nights did not help, either,” said Sakeenah, whose symptoms included vomiting, sometimes throughout the day, and sensitivity to light.

Sakeenah, whose mother and son also suffer from migraines, has consulted a neurologist and undergone an MRI, with results coming back negative. In addition to Western medication, she has tried acupuncture and ayurvedic treatments but she admitted that she did not follow up with her sessions.

“I just want fast relief because it’s so debilitating and I want it to go away, especially when I need to work,” she said, adding that stress is a main trigger for her.

Many possible causes

Another word for headache is cephalalgia, which originates from an ancient Greek word meaning “head pain”.

A headache is not a disease per se, but a medical problem that is often a life-long condition. Among the most common disorders of the nervous system, headaches occur due to many possible causes.

Some people are more prone to headaches because of their genetic make-up, brain chemistry and also hereditary reasons. Lifestyle is another factor, with stress, lack of sleep and missing meals among the causes.

Understanding headaches starts with knowing that there are many different types of headache disorders.

Each experience differs in terms of nature of pain (dull, sharp, throbbing and so on), severity, pain location, frequency and duration of attacks, and presence of other symptoms, such as nausea.

The most severe form of headache is a cluster headache, which is almost exclusive to middle-aged men.

Studies also show that over 80% of women in their reproductive age have headaches, usually more severe than men. Women also suffer more from migraines and tension-type headaches than men. Fluctuating hormone levels as women go through menstruation, pregnancy and menopause may play a role, but it does not apply to all women.

According to the World Health Organisation (WHO), the most common headache disorders are tension-type headache (TTH), migraine, cluster headache and “chronic daily headache” syndromes.

Studies show that headaches disrupt family and quality of life, and productivity, more than many other chronic disorders.

According to WHO, in developed countries, TTH alone affects two-thirds of adult males and over 80% of females. In Malaysia, no major studies have been done on the prevalence of headaches.

“However, headache is the most common complaint in our neurology outpatient clinic and we see about 12 new patients a week,” said Prof Datuk Dr Raymond Azman Ali, senior consultant neurologist at Hospital Universiti Kebangsaan Malaysia’s Department of Medicine.

The most common form of headache the clinic encounters, he added, is migraine, although many medical books list tension headache as most common.

“The reason for this discrepancy is, I believe, that the majority of tension headache sufferers prefer to buy analgesics over the counter or dispel their headache as something normal, since they have lived with it for many years.

“Another reason is that a lot of them see a general practitioner or their family doctor and are not referred to us,” said Dr Raymond, who is also Malaysian Society of Neurosciences’ Epilepsy Council chairman.

He said those aged between 12 and 35 form the biggest pool of people who suffer from headaches, and most are female.

One of the biggest myths about headaches is that they are due to brain tumours.

“Our brains do not have pain receptors. You can push a needle through the brain and you will feel nothing. The pain receptors are in the meninges (brain covering), blood vessels and skull.

“Hence, a brain tumour has to be large enough to compress these pain-sensitive structures to cause a headache. But by then, patients would have suffered epileptic seizures, hemiparesis (weakness or paralysis in one side of the body), dysphasia (a communication impairment resulting from brain injury) or a personality change,” explained Dr Raymond.

Some people also wonder if sex causes headaches.

“(That is true). It is well-known that some patients have what is called post-coital headaches,” he said.

However, the myth that hypertension causes headaches is not true.

“If someone with hypertension has headaches, he or she probably has two unrelated problems, a phaechromocytoma (adrenal tumour secreting hormones produced by the adrenal glands, which is rare) or malignant hypertension (very high blood pressure of over 220/140 mmHg). So, don’t wait until you have a headache before checking your blood pressure.”

Knowing the triggers

Tension-type headaches may be triggered by poor posture, working in awkward positions, stress, depression and anxiety.

For migraines, triggers include certain foods, hunger, changes in weather or sleep patterns, exposure to bright lights or unusual odours, skipping a meal, drinking alcohol and having sex.

Dr Raymond said that although certain foods trigger migraines, not everyone experiences the same thing.

“Therefore, we do not ‘ban’ these foods but rather, ask patients to note down in their headache diary what they ate prior to an attack, like durian, chocolate, milk, peanuts, coffee, wine, citrus foods or certain drugs (such as nitrates for heart patients).

“Surprisingly, over-relaxing is also a recognised factor with migraines. Others include sleep deprivation or too much sleep, travelling, heat, unaccustomed exercise, menses and irregular meals, probably related to hypo-glycaemia,” he said.

For women with migraine, the trigger is often linked to a drop in estrogen levels.

According to the book Mayo Clinic On Headache by Jerry W. Swanson, an estimated 50% of migraineurs may have a magnesium deficiency.

For cluster headaches, there are no known triggers. However, measures that may help avoid an attack include having a regular sleep schedule, and avoiding afternoon naps, alcohol, tobacco products, glare and bright lights.

Although most headaches are not life-threatening, they can be debilitating.

“It is not severe in the sense that it can kill you or cause permanent disability. As a neurologist, I want my patients to lead a normal life as much as possible.

“Untreated migraine can lead to depression and about 18% of migraine sufferers have suicidal ideation,” said Dr Raymond, quoting a past study done in Malaysia between Universiti Kebangsaan Malaysia Medical Centre, Universiti Malaya Medical Centre and Hospital Kuala Lumpur,

Dr Raymond cautioned about secondary headaches, which are more dangerous and can lead to death.

“These are headaches due to an underlying disease such as meningitis, temporal arteritis, cervical spondylosis, dental problems, glaucoma or SLE (systemic lupus erythematosus).”

So when should people suffering from headaches seek medical attention?

According to Mayo Clinic On Headache, if you experience a sudden, severe headache that is unlike any you have had before, or a headache that continues to worsen over time, see a doctor promptly.

The same applies to a headache that comes with a high fever or stiff neck.

“I think (you should seek medical advice) all the time, unless it is acute and accompanied by unequivocal flu-like symptoms. A viral upper respiratory tract infection is the most common cause of short-term headache.

“All other headaches should be treated seriously. Let us decide whether to investigate or not. Generally, I don’t do any investigations if the patients have typical features of a primary headache,” said Dr Raymond.

These days, many people are keen to seek other forms of treatments for their headaches besides relying on medication. These treatments are either used as alternative or complementary therapies.

“There is some evidence to support acupuncture’s role in migraine and tension headaches. See a neurologist first and then decide what to do next,” advised Dr Raymond.

A 17-day study of coral reefs off the Sabah east coast Semporna district has reaffirmed that the area has the world’s highest marine biodiversity

Sabah rules in sea life

KOTA KINABALU: A 17-day study of coral reefs off the Sabah east coast Semporna district has reaffirmed that the area has the world’s highest marine biodiversity.

But the study involving Malaysian and Dutch researchers unearthed some disturbing information – the area is under serious threat with only 5% of coral studied deemed in excellent condition and another 23% in good condition.

The study was jointly organised by WWF-Malaysia, the Netherlands Centre for Biodiversity (NCB) Naturalis, Universiti Malaysia Sabah and Universiti Malaya.

Under the sea: Dr Hoeksema holding a rare species of mushroom coral (Halomitra clavator) in the Semporna Priority Conservation Area.

Expedition co-leader Dr Bert Hoeksema of NCB said the Nov 29 to Dec 18 study found a staggering 43 mushroom coral species in waters off the 50 islands off Semporna.

He said the previous recorded richness of this family was 40 species at several sites off Sulawesi, Indonesia and Papua New Guinea.

Dr Hoeksema said the study found 844 fish species and more than 90 coral shrimp species, rivalling the highest counts in the Philippines and Indonesia.

Dr Hoeksema disclosed this yesterday when releasing preliminary results of the expedition.

But he said researchers found that nearly all the areas studied had been impacted by damaging human activities – fish bombing, discarded fishing gear and solid waste.

He said researchers also heard 15 explosions due to fish bombing and found several unexploded fish bombs “that were just a few days old”.

Dr Hoeksema said the researchers found discarded fishing nets on the coral “and these were like death blankets over the reefs”.

WWF Malaysia chairman Datuk Tengku Zainal Adlin Tengku Mahamood said the expedition had showed the waters off Semporna as “amazingly right in comparison to the other hotspots for marine diversity in the Coral Triangle”.

“We cannot save what we do not love, and we cannot love what we do not know.

“We have now increased our knowledge of the rich reefs in Semporna and we must work harder to save them,” he said.

Videos of the expedition can be viewed on YouTube at www.youtube.com/user/2010SMEE.

IPOH: One family's worst nightmare took a surprising twist when they were informed that their mother, initally listed as among those killed in the Cameron Highlands bus tragedy, was actually alive albeit in critical condition.

Anusorn Sukhumalsakul, 40, had arrived at the Raja Permaisuri Bainun Hospital here late on Monday after receiving news of the 'death' of his 59-year-old mother, Darunee Songserm, and to claim the remains.

But all his grief turned to relief when he was told that Darunee was alive but critically ill in the intensive care unit.

"It feels like my mother has been given a second chance to live," he said at the hospital Tuesday.

Due to difficulties in identifying the victims due to the injuries, the hospital had added Darunee's name to the list of those who died.

The authorities informed the family as soon as the hospital could not determine Darunee's identity.

Darunee suffered serious injuries to her head and chest in Monday's bus accident in Cameron Highlands.

Camerons crash: Probe report ready in a fortnight

Published: Tuesday December 21, 2010 MYT 8:56:00 PM
Updated: Tuesday December 21, 2010 MYT 9:12:13 PM

KUALA LUMPUR: The investigation paper on the double-decker express bus crash at KM15 of the Cameron Highlands-Simpang Pulai Road which killed 27 people, including 25 Thai tourists, is expected to be ready in two weeks.

Malaysian Institute of Road Safety Research (Miros) managing director Prof Dr Ahmad Farhan Mohd Sadullah said a detailed analysis of the crash was being done with the cooperation of other agencies like the police and Public Works Department.

"The probe is still in the early stage and the outcome is only expected in about two weeks' time as usually the case," he said Tuesday.

He, however, said that according to the standards, double-decker buses were not suitable for negotiating roads on hilly terrain.

"On a sloping road, the double-decker which is higher than normal buses, is not in a very stable condition and may pose a danger to the passengers. This type of bus is more suitable for flat roads."

Ahmad Farhan said bus operators should therefore take the suitability and conditions of buses into consideration before ferrying passengers to their destinations.

Bukit Aman Traffic chief Datuk Abdul Aziz Yusof said all aspects, including the condition of the bus and road were being covered in the police investigation into MOnday's horrific accident.

"At this moment, we should not be pointing fingers at anyone until the results of the investigation are known," he said.

The Pan Malaysia Bus Operators Association has urged the Government to hold short-term courses for bus drivers to improve their driving skills, and also on providing emergency first-aid and in fire-prevention.

Its president Datuk Mohamad Ashfar Ali said the Government must also look at how driving schools were teaching those applying for the bus driving licence, to ensure they would become efficient drivers, hence reducing road accidents and fatalities.

He also urged the Road Transport Department to place warning signs at the "black spots" or accident-prone road stretches.

Meanwhile, the Land Public Transport Commission (LPTC) is making road safety a high priority area in its list of concerns as it moves towards full operation next year.

Its chairman Tan Sri Syed Hamid Albar in a statement said LPTC had begun discussions with bus operators and relevant government agencies with the view to resolving issues that resulted in safety being compromised within the industry.

He added that LPTC would be drawing up policies, rules and regulations to ensure that safety standards of public transport services were maintained. - Bernama

IS THERE A CURE FOR COLD - All of us suffer from the common cold at some time or other. Is there a cure for it? Or is there even good relief for it?

Is there a cure for cold?

All of us suffer from the common cold at some time or other. Is there a cure for it? Or is there even good relief for it?

NEW YORK: The largest study of the popular herbal remedy echinacea finds it won't help you get better any sooner.

The study of more than 700 adults and children suggests the tiniest possible benefit - about a half-day shaved off a weeklong cold and slightly milder symptoms. But that could have occurred by chance.

For most people, the potential to get relief a few hours sooner probably isn't worth the trouble and cost of taking the supplement, researchers said.

With no cure for the common cold, Americans spend billions on over-the-counter pills, drops, sprays and other concoctions to battle their runny noses, scratchy throats and nagging coughs. Some turn to echinacea, a top seller marketed as a product that helps the immune system fight infections.

In the past, some studies found it did nothing to prevent or treat colds; others showed modest benefit. Research on echinacea, also known as purple coneflower, is hard to compare because there's more than one kind of plant, and different parts of it are used.

With government funding, Dr. Bruce Barrett and colleagues at the University of Wisconsin tackled the question again, using newspaper ads and posters to find volunteers with colds in the Madison, Wis., area.

The participants ages 12 to 80 were randomly assigned to get echinacea tablets, a dummy pill or no treatment at all. Those who got the herb took the equivalent of 10 grams of dried echinacea root the first day and 5 grams the next four days.

Twice a day, they graded their symptoms until their cold was gone.

From those scores, the researchers saw a trend toward shorter and slightly less severe colds for those taking echinacea compared to those who didn't. However, the results did not reach statistical significance, meaning they could have occurred by chance. There were no apparent side effects from the echinacea.

Barrett and other experts said the findings would probably be viewed as positive by echinacea supporters but as the "nail in the coffin" by critics.

"It's not a compelling result in either direction," said Mark Blumenthal, executive director of the American Botanical Council, which follows research on herbal products. He said Barrett is on the group's advisory board.

Blumenthal said the study was well designed, used a good quality product at a reasonable dosage and tested echinacea in a real-world setting, rather than giving colds to research volunteers.

The study's findings were released Monday by the Annals of Internal Medicine. The authors include the co-founder of the Australian company that provided the echinacea for the study, but he was not involved in the research.

It was funded by the National Center of Complementary and Alternative Medicine, part of the National Institutes of Health. The center, set up to test herbs and other alternative health remedies, has spent $6.8 million testing echinacea since 2002.

The center's director, Dr. Josephine Briggs, said there are no plans to support more human research on echinacea.

"I think what we're seeing is pretty clear. If there's a benefit of echinacea, it's very modest, Briggs said.

So what's a cold sufferer to do?

"There's nothing that's going to make it go away," said Dr. Ronald Turner, of the University of Virginia School of Medicine. He led another government-funded study of echinacea that had negative results.

Some over-the-counter medications ease the symptoms a bit, he said. Americans spent $5.3 billion on cold and cough remedies at major U.S. stores in the year ending Nov. 27, according to The Nielsen Co. (Cold and cough medicines are no longer recommended for children under 4.)

And don't go running to the doctor for antibiotics, said Dr. Ann Falsey of the University of Rochester Medical Center. Antibiotics don't work against cold viruses and can have side effects.

Rest, fluids, pain relievers and over-the-counter medications to relieve symptoms are generally recommended. - AP

Pemandu baru 3 bulan kendali bas - Omar Shahidan yang terbunuh bersama sebahagian besar pelancong Thailand, di Kilometer 15 Jalan Cameron Highlands...

Pemandu baru 3 bulan kendali bas

Turut maut dalam nahas berkenaan, pemandu pelancong, Soontorn a/l Kian, 38 tahun.

Turut hadir, Pengarah LPKP Sabah, Yozimah Jasni.

An eerie atmosphere hung over the Raja Permaisuri Bainun Hospital mortuary as the bodies were brought in by two vehicles

    Eerie silence at hospital mortuary

IPOH: An eerie atmosphere hung over the Raja Permaisuri Bainun Hospital mortuary here as the bodies were brought in by two vehicles.

At 2.35pm, a police truck arrived with nine, followed by a Civil Defence Department truck with 12 bodies some 90 minutes later.

Those present at the mortuary fell silent as the black body bags were brought down from the vehicles.

Grim police, hospital and funeral parlour representatives helped to wheel the bodies into the mortuary.

Hospital staff also promptly began with post-mortem work.

As the day progressed, the remains of those who had survived the crash but died at the hospital, were brought to the mortuary.

“Never have we received so many bodies in a single day,” said a mortuary staff.

As at 8.30pm, the mortuary was still crowded with people as post-mortems continued.

Tourism Ministry officials were at hand to help the victims’ families.

Expressing his sadness over the tragedy after visiting the injured at the hospital, Perak Mentri Besar Datuk Seri Dr Zambry Abdul Kadir said the state crisis response team would be investigating the accident.

He added that the state would also look into the possibility of getting accommodation for families of the victims.

He said Thai Embassy minister counsellor Somphong Kang Thong had informed him that the families would be flying in from Bangkok today.

Doctors and nurses at the emergency ward of the Raja Permaisuri Bainun Hospital scrambled to save lives after the horrific bus crash

    Docs scramble to treat survivors

IPOH: Doctors and nurses at the emergency ward of the Raja Permaisuri Bainun Hospital here scrambled to save lives after the horrific bus crash.

There was blood everywhere on the floor as survivors of the double-decker bus crash near Simpang Pulai were given immediate medical attention.

Victims were being transferred in and out of the critical ward but doctors and medical officers declined to divulge information about the condition of their patients.

There were no family members seen at the mortuary as most of the victims were Thai nationals, whose families had yet to arrive.

Pressmen crowded the front of the mortuary to find out the number of victims involved in the accident.

The list of names of the deceased kept growing every hour as more bodies were identified.

First Secretary to the Royal Thai Embassy in Malaysia, Suwit Mangkhala, expressed his deepest regret over the accident.

He said they were trying to verify the identities of the Thai nationals and inform the Foreign Affairs Ministry in Bangkok.

“The families of victims have been contacting us and they are very worried,” he said, adding that the embassy was doing its best to provide information to the family members.

Iuntnapa Chuchart, 53, who is being warded at the hospital for multiple lacerations on her face, said the group had arrived from Bangkok on Dec 18.

The younger brothers of tour guide Soontorn Kian, 38, arrived at the mortuary at about 10pm.

“We will be taking his body back to our hometown in Alor Setar,” said Prishit Kian, 35, who came from Penang with another brother, Pranom Kian, 25.

Nahas paling ngeri - Percutian sekumpulan 35 pelancong warga Thailand ke pusat peranginan Cameron Highlands, Pahang bertukar menjadi tragedi...

    Nahas paling ngeri

Worst road accident in Malaysian history - 27 killed in Camerons crash: Thai holidaymakers die after bus hit divider and overturned

    27 killed in Camerons crash

Thai holidaymakers die after bus hit divider and overturned

IPOH: Twenty-seven people were killed when their bus hit a divider and overturned on its way down from Cameron Highlands in what is possibly the worst road accident in Malaysian history.

The tragedy at KM15 of the Cameron Highland-Simpang Pulai road claimed the lives of 25 Thai nationals who had been on a holiday in the hill resort.

Perak deputy police chief Senior Asst Comm Datuk Zakaria Yusof said the other two casualties were driver Omar Shahidan, 48, and tour guide Soontorn Kian, 38. Both were Malaysians.

    source: thestaronline Channel

“There were a total of 37 people onboard at the time, 34 were Thai holidaymakers from Bangkok and three others Malaysians,” he said.

The double-decker bus was heading from Cameron Highlands towards Kuala Lumpur at 11.40am yesterday.

“The driver was trying to negotiate a sharp bend but failed, causing the bus to hit a divider before overturning.

“The bus landed on its roof in a ditch about 51m away from the divider,” SAC Zakaria said.

He said the impact of the crash was so severe that bodies were flung several metres away from the wreckage while others were trapped deep in the mangled vehicle.

Twenty-two of the victims were killed on the spot. Five others died after being rushed to the Raja Permaisuri Bainun Hospital here.

SAC Zakaria said the cause of the accident could have been due to speeding or technical failure of the brakes.

It took about 100 personnel from the police, Fire and Rescue Department, Civil Defence Department and Public Works Department over three hours to retrieve the bodies.

The wreckage was finally towed downhill at about 4pm, causing massive jams on both sides of the road as far as 5km away.

It is learnt that of the 10 survivors, one was discharged after being treated for minor injuries; three were undergoing surgery and the rest warded as at 7pm.

SAC Zakaria said the bus was heading to Kuala Lumpur where the Thais were believed to have been scheduled to fly back to Bangkok.

The Cameron Highland-Simpang Pulai Road, which has many curves and gradients, was opened to traffic in 2003.

It is an alternative road to the highlands besides the Tapah route.

Setelah Galang dan Kirana bersatu kembali mereka bernikah dan dua tahun kemudian Kirana mengandung anak pertama mereka. Sewaktu Kirana hendak bersalin Galang tiada di sisinya. Raka menghantar Kirana ke rumah sakit. Kirana melahirkan seorang anak lelaki. A happy ending... Tamat.

    Kirana: Galang..
Galang: Maafkan aku, Kirana... Tadi aku tak dapat bersama kau tapi aku cuba balik secepat mungkin. Untuk kau dan anak kita.
    Galang: Kacaknya macam papanya.
    Kirana: Aku gembira, Galang.
Raka: Nasib baik kau lewat. Kalau tadi kau ada bersama Kirana masa dia bersalin, kau tentu lebam-lebam macam aku.
Kirana: (Sambil tersenyum Kirana tanya Raka adakah dia menyindir) Kau menyindir, Raka?
    Raka: Taklah cuma sedikit...

Nenek Aida: Udah.. udah.. udah, sekarang begini saja, siapa yang nak beri nama bayi ini. Nenek Aida saja ya?
Nenek Sita turut mahu menamakan anak Kirana dan Galang. Tetapi kata Raka..

Raka: Tapi kalau dalam filem, anak yang baru lahir diberikan nama sempena nama penolongnya. Jadi bagaimana kalau beri nama dia Raka Evandra?

Galang: Tunggu... tunggu.. tunggu..
